不同护具对FAI者扭伤高危动作下肢运动学和 动力学特征的影响
摘要:
[目的]旨在分析2种护踝对功能性踝关节不稳(functional ankle instability,FAI)者执行快速变向着陆等动作时,其下肢运动学和动力学特性的影响,期望能够揭示不同踝关节护具在预防踝关节扭伤方面的保护机理。[方法]纳人10名功能性踝关节不稳者为测试对象,通过Nokov Mars2H红外光点3维运动捕捉系统和Bertec3维力测台获得受试者分别佩戴半刚性护踝、弹性护踝和不佩戴任何护具时随机完成急停侧切、急停起跳落地及单脚跳箱落地动作的运动学与动力学特征。采用3X3重复测量方差分析验证护具和动作对FAI者下肢运动学和动力学特征的影响。[结果]动作和护踝仅对FAI者垂直GRF峰值有显著性交互作用(Fu.=16.87,P<0.001)。与不佩戴任何护具相比,弹性和半刚性护踝可减小FAI者着地时刻踝关节跖屈角(P<0.001)、内翻角(P<0.001,P=0.001),额状面ROM(P=0.001,P<0.001)、膝关节内旋角(P=0.005,P=0.002)和垂直GRF峰值(P<0.001),增大膝关节屈曲角(P<0.001,P=0.002)和垂直方向TTP(P<0.001)。相比半刚性护踝,弹性护踝可减小踝关节额状面ROM(P0.002)和垂直GRF峰值(P<0.001),增大膝关节屈曲角(P<0.001)和垂直方向TTP(P=0.004)。[结论]FA1者佩戴弹性或半刚性护踝时,均可加强踝关节稳定性,与半刚性护踝相比,弹性护踝的防护效果更好。
[Obective]The study aimed to investigate the lower extremity biomechanical characteristies of dilerent ankle braces on functional ankle instability(FAl) individuals during rapid directional changes and landing maneuvers related to ankle sprains, in order to gain comprehensive understanding of the proteetive mechanism for ankle braces, [Methods] 10 FAl participants were randomly selected to wear semi-rigid ankle brace, elastic ankle brace and no ankle brace when performing side-cutting, stop jump landing and single-leg drop jump from box using a random number generator for assignment, 3D motion capture system( Nokov Mars2H) and force plate(Bertec) were used to obtain kinematic and kinetic data for subjects. A two-way repeated measures 3X3 ANOVA was used to verify the effects of ankle brace and maneuvers on lower extremity kinematic and dynamic characteristics of the lower limbs in individuals with FAl. [Results]The results showed a significant interaetion between maneuver and ankle brace on peak vertical GRF for FAl individual(F[1,4]=16.87, p<0.001), Compared with no ankle brace, elastic and semi-rigid ankle braces reduced ankle plantarflexion angle(P<0.001), inversion angle(P<0.001, P=0.001), frontal plane ROM(P =0.001, P<0.001), knee internal rotation angle(P=0.005, P=0.002) and peak vertical GRF at initial contaet(P<0.001), inereased knee flexion angle(P<0.001, P=0.002) and vertical TTP(P<0.001), In addition, compared with the semi-rigid ankle brace, the elastie brace reduced ankle frontal ROM(P=0.002) and peak vertical GRF(P<0001) and inereased knee fexion angle(P<0.00l) and vertical TTP(P=0.004). [Conclusion]When FAl wears elastic or semirigid ankle protection, it can strengthen the stability of ankle joint, Compared with semi-rigid ankle braces, elastic ankle braceswas more prominent.
